Care Team

At Hospice of Douglas County, care is provided by a team of health care professionals who are skilled at serving people with life-limiting illnesses. Our hospice team works together to meet the medical, emotional and spiritual needs of patients and their families and are devoted to providing compassionate care to every patient.

Physician
We encourage and include the patient’s personal physician in our team approach and in planning patient care. The physician will work in collaboration with our Medical Director to assure patients receive excellent care.

Registered Nurses
Registered nurses provide skilled nursing visits, support and teaching and coordinate all aspects of the patients care. A registered nurse is available 24 hours a day, 7 days of the week. They keep the family and primary physician informed of any changes. Additionally, our registered nurses provide support and valuable information to family members on how to care for their loved one.

Social Worker
The Social Worker addresses emotional and psychological needs by providing comfort and support to patients and family members who are dealing with the stressful issues related to a life-limiting illness. The Social Worker also lends support with non-medical concerns, such as legal and financial issues, access to community resources and future planning.

Chaplain
The Chaplain offers spiritual support directly or in collaboration with the patient and family’s clergy or spiritual counselor. Our chaplains honor the diversity in religious beliefs and backgrounds of all of our patients.

Hospice Aides
Hospice aides provide individualized, hands on care for patients under the direction of the Registered Nurse. Hospice aides assist with day-to-day personal care, such as bathing and grooming. They may also assist with light house cleaning, meal preparation and providing respite for caregivers.

Medical Director
The Medical Director works with the patient’s physician and the hospice team to develop a care plan that meets the patient’s specific needs.

Massage and Music Therapy
These therapies are available to patients to promote comfort and improve quality of life. They are provided by a board certified music therapist and a licensed massage therapist.

Bereavement Team
Bereavement support is offered by our team to address the phases of grief and acceptance. Our bereavement team provides this service to families for at least 13 months after the patient has died.

Volunteers
Our compassionate and qualified volunteers provide assistance to patients and their families by offering comfort, companionship and respite care.

Physical, Occupational and Speech Therapists
These additional therapies may be utilized based on the individual needs as warranted for specific conditions and patient goals.
